Maid Mode Sakuyamon is a God Man Digimon. It is a form Sakuyamon takes when it performs Shinto rituals and listens to God's will. It seldom fights in Maid Mode (巫女形態 Miko Keitai?, lit. "Miko Mode"), but has strengthened its ability to exorcise evil spirits.[4]

- Izuna[5] (飯綱?): Attacks the opponent with the four kuda-gitsune carried on its waist.
- Kongoukai Mandara (金剛界曼荼羅? lit. "Diamond Realm Mandala"): Strikes the ground with the Kongou Shakujou, spreading a purifying barrier that exorcises evil spirits.
